2 great fun aspects of DAoC(dark age of camelot) pvp were:
Keeps - several keeps that could be captured by any 'side', in our case guardians or defiants, and then owned and maintained by a guild. each keep you own gives a bonus to all players in your faction(5% damage/healing, 5% xp, that sort of thing). Once captured a keep will spawn npc guards and a lord. these can be upgraded by the guild that claimed the keep. This guild will be notified when the keep is under attack to give them a chance to call help from thier faction for all players to join in the defence or attack.
Darkness falls - whichever faction owns the most keeps will also gain access to a world dungeon with pve mobs and bosses. Only one faction can enter the dungeon at a time, but the pvp aspect tended to occur when control of DF was lost and the new 'owners' would come pouring in. Any players already in DF can stay there until they are killed and have to res to graveyard. So huge running battles would occur as the new owners tried to purge the enemy players from the dungeon.
wishful thinking i know. Plus the pvp/pve open world gear disparity issue must be resolved before any openworld stuff can be implemented..
